Wellness Visits for Medicare Patients

Medicare has some precarious policies with paying cash for physical therapy for a non participating Medicare provider. If you want to come to Dr. Steph Johnson on your own free will through a self referral, you must request up front in our initial phone call that you do not want to bill Medicare for any of the services you receive. Dr. Johnson can only accept Medicare beneficiaries as patients when the patient does not want Medicare billed for any PT services.

There are many instances where traditional physical therapy through a Medicare Provider will suffice, Dr. Johnson will be happy to discuss with you when it might be a better option to go that route. 

Pricing: 1-hour wellness visit: $150, 30-min wellness visit: $75

In-home visits are available within a 30 minute radius of Personal Pilates Plus. This is ideal for patients with dizziness, or PTSD surrounding driving after MVA. Please call to schedule in home visits. An additional $50 travel charge will be applied to above pricingNote: travel fee is waived for wheelchair users. 

Out of Network Physical Therapy
A superbill can be provided to submit to your insurance for out-of-network reimbursement. It is recommended to call your insurance company ahead of time to find out if you have any “out-of-network” reimbursement or application toward your deductible benefits for expenses sent in via self claims.

HSA and FSA cards are accepted, and an itemized receipt can be generated for proof of appointment. 

What to ask your insurance about using your Out-of-network benefits:

1. Do I have out of network coverage?

2.What is my cost share?

3. Can this be applied towards my deductible?

4. What are the steps to file my out of network claim after my physical therapist provides me with a superbill?

5. Do I need a referral? (Colorado is a direct access state) Payment accepted: cash, check made out to Steph Johnson PT, credit or debit card, and HSA or FSA cards Cancellation Policy I have a very understanding and empathetic nature when it comes to last minute emergencies.  If you need to cancel an appointment, I ask that you give at least 24 hours advance notice.  If you are unable to provide a notice of cancellation, a $60 fee will be applied.
